Lower back surgery is the last choice unless it’s an emergency. Most disc herniations and stenosis improve with conservative treatment. Deciding on surgery without attempting sufficient conservative treatment is not recommended.

When Surgery Is Needed (Emergency)

  • Loss of bladder/bowel control (cauda equina syndrome)
  • Rapidly progressing bilateral leg paralysis
  • Rapidly worsening muscle weakness

→ In these cases immediate surgery is needed.

When Conservative Treatment Should Be Tried First

  • Severe pain but no paralysis
  • MRI shows abnormality but symptoms are not severe
  • Have not tried systematic conservative treatment for 3-6 months
  • Daily activities are still possible

Check if you’ve tried systematic conservative treatment including medication, injections, physical therapy, and movement correction for at least 3-6 months.

Do Imaging and Symptoms Match?

Need to confirm if MRI findings match actual symptoms. Even with imaging abnormalities, the cause could be elsewhere.

Surgery Can Recur Too

Surgery resolves structural problems, but if underlying causes (posture, muscle imbalance, movement patterns) aren’t improved, it will recur. Rehabilitation is essential even after surgery.
