Your Face Burns, But Every Test Comes Back Normal
A steady burning ache is a different problem from a sudden electric jolt. It points to the nerves.
Some of the nerve fibers that create pain are so fine that standard tests cannot detect them at all.
When the fine fibers of the trigeminal nerve are damaged, the burning continues even though nothing abnormal is visible.
Swollen neck muscles press on the nerve, and the longer that pressure lasts, the more the nerve can be damaged.
A normal test result does not mean the nerve is intact.
When We Look at the Nerves
- Your cheek, forehead, or jaw burns and stings
- The burning runs steadily through the whole day
- The painful area actually feels numb
- A collar brushing your skin is enough to hurt
- Examinations turned up nothing at all
Why the Tests Miss It
Nerve fibers come in different sizes.
Large fibers show up on nerve conduction studies, but the finest fibers that carry burning and stinging never register on that test.
A Brake Comes Off
The fibers that sense cold normally hold down the signals sent by pain fibers. When those fibers are damaged first, the restraint disappears and only the burning is left.
Numb and Painful at Once
The area with reduced sensation is often the area that hurts most. It sounds contradictory, but it is a common pairing in nerve damage.
There is a reason this pain becomes more common around menopause.
The hormones that protect the fine nerve fibers decline, leaving those fibers more vulnerable to damage.
Facial Pain, So Why Examine the Neck
When the neck muscles swell, the nerves running between them get compressed.
Brief pressure ends as a dull ache.
Pressure that lasts damages the nerve itself, and that is when the pain turns to burning.
The trigeminal nerve that carries facial sensation and the nerves coming up from the neck pass through the same point in the brainstem.
That is why a problem starting in the neck is felt in the face.
If pressing the back of your neck or turning your head changes the facial symptoms, the neck is involved.
If the ache sits around the eye as well, the nerve may not be damaged yet. Facial and eye-area pain covers that stage.
Burning on the tongue belongs to the same system.
Sensation in the tongue and mouth is carried by the same trigeminal nerve that serves the face.
A change in how food tastes is explained here as well.
See a doctor immediately for any of these
- Weakness or complete loss of sensation on one side of the face
- Pain with fever and blisters, a possible sign of shingles
- Sudden blurring or loss of vision on one side
- Pain that wakes you from sleep
- Weight loss together with worsening pain
- Over age 50 with a tender scalp and jaw pain when chewing
Our Approach at Yonsei SM Pain Clinic
We do not look at the face alone.
We reduce the load coming from the neck, which meets the facial signals in the brainstem.
We bring the swelling down and quiet the signals rising from the neck
Guided by ultrasound, we treat the tender points in the neck and temple muscles with injection therapy.
We wake the neck muscles up and rebuild their endurance
A dedicated therapist works one to one to release the tight muscles and restore movement in the upper cervical joints.
Make the change stick, with the goal of lowering the risk of recurrence
We correct the forward head posture and the jaw clenching habit together.
※ Responses to injection and rehabilitation therapy vary between individuals, and temporary soreness or bruising can occur.
The treatment plan is decided after consultation.
Worth Bringing With You
- Results of any recent tests
- A list of your current medications
- A note of when the pain peaks
Frequently Asked Questions
Every test is normal, so how can this be a nerve problem?
MRI and nerve conduction studies examine structure and large nerve fibers. The fine fibers that create burning do not register on them. The normal result and the source of the pain are two separate things here.
Do you also look at burning inside the mouth?
Sensation in the tongue and mouth is carried by the same trigeminal nerve that serves the face. Because signals rising from the neck act on the point where that sensation converges, we check the neck as well.
Will treating my neck make the facial burning go away?
It changes to the extent that the neck is involved. If the facial symptoms shift when you press the back of your neck or turn your head, there is considerable room to work with the neck.
